A clear, clinically supported referral pathway

Cannxtions provides healthcare professionals with a straightforward pathway for referring patients who may benefit from further assessment and an alternative approach to care.

Referred patients receive an approximately 40-minute, face-to-face consultation with an experienced healthcare practitioner. The consultation includes individual assessment, education and ongoing follow-up where appropriate.

What your patient can expect

  • A comprehensive face-to-face consultation

  • Care delivered through a structured clinical pathway

  • Individualised education and ongoing support

  • Communication back to you as the referring practitioner, where appropriate

Once a referral is received, a member of the Cannxtions team will contact your patient within 48 hours to discuss the next steps.
